When I first read the AFFIRM trial paper, in 2004 (mentioned in Susan's linked editorial), my thought was the analysis was bogus as the results were based on "intention to treat." Meaning, if you were in the rhythm control arm, your results were included there, even if you did not successfully maintain NSR. Mark, Great report! The one thing I know I can do to cause an episode is to train/ski/bike or whatever at too hard an exertion level for too long. Everyone has to find their own limits. I think Zone 2 is pretty safe for many. One of the things I work on optimizing is blood sugar. Exercise at a Z2 level will open up GLUT 4 pathways for glucose intake into the cells without insulin. For me, I've had times when I can't identify triggers and when I can. When I have my electrolytes optimized, triggers are few and the primary one is overdoing exercise meaning too long a duration at too high an intensity (creating a delayed vagal trigger).
